State recommends Cedarcreek get cut off by Medicare, Medicaid

WARREN — The Ohio Department of Health has issued a notice to CedarCreek Health & Rehabilitation that it is not in compliance with the requirements of Medicare and or/Medicaid programs.

Following a survey of the nursing home on Tod Avenue, the department has sent a list of remedies to the Center for Medicare and Medicaid Services following inspections made at Cedarcreek.

The remedies, or enforcement actions, include state monitoring effective immediately; discretionary denial of payment for new Medicare and Medicaid admissions effective Feb. 8; transfer of residents; and discretionary termination of the facility’s Medicare and Medicaid provider agreement effective no later than Feb. 18.

Tennessee-based New Beginnings Healthcare and Rehab, which also operated Campus Health Care Center at 196 Colonial Drive in Liberty, shut down the Liberty facility in later January after the state health department proposed to revoke the license there.

New Beginnings has filed for bankruptcy.
