Grace Woods Senior Living, LLC, an assisted-living facility with locations in Niles and Salem, recently promoted Sara Meade to comprehensive health care coordinator.

Meade’s duties include managing and coordinating medical care for all assisted-living residents. Meade, a practicing licensed practical nurse with extensive long-term care experience, is pursuing her professional nursing degree to become a registered nurse.

Grace Woods Senior Living provides services for young, middle-age and elder adults who require assistance with activities of daily living. An adult registered at Grace Woods receives individualized care planning, coordinated care management and skilled nursing.

Grace Woods offers nursing observations, medication administration, meals and snacks, barber and beauty services, on-site physical therapy, occupational therapy and speech therapy services. Recreational activities and social events also are available.

Grace Woods Senior Living, LLC, an Ohio Department of Health licensed residential care facility that is certified by the Ohio Department of Aging, has two assisted-living facilities in the Mahoning Valley: 730 Youngstown-Warren Road in Niles and 1166 Benton Road in Salem.

Grace Woods also provides outpatient rehabilitation services through its company, Integrated Physical Therapy Services Inc., located in Niles and Hubbard.
