The sanction recommended by an Ohio Department of Health hearing officer could shut SCOPE Inc. down, said Ralph Smith, who has been in charge of the agency for senior citizens in Trumbull County.

After a hearing in Columbus in June, an examiner recommended a permanent revocation of SCOPE’s Provider Certification as a Long-Term Care-Agency.

SCOPE has held the certification since 1991.
