HUBBARD SCHOOLS Open enrollment

Hubbard schools will have open enrollment during the 2004-05 school year. Eligibility and application procedure:
Tuition will not be assessed to pupils; the state reimburses the district. Limits will be set by class size.
Transportation is a parental responsibility.
Pupils will not be admitted if they have been suspended for any reason.
Special-education pupils may not be accepted if programs are not available.
Applications can be picked up from 8 a.m. to 3:30 p.m. at the school board offices at Reed Middle School. Filing deadline is 3 p.m. May 14. Applications are not to be prepared when picked up.
One application must be submitted for each pupil requesting transfer.
Application must include immunization records, birth certificate with embossed seal, Social Security number and custody papers, if applicable. The information will be retained for school records.
Notification of acceptance or denial will be by mail. No information will be given over the phone.
