HUBBARD -- The Hubbard Post Office now accepts passport applications from 8:30 a.m. to 3 p.m. weekdays and 8:30 to 11 a.m. Saturdays.
Applicants must produce a birth certificate, two passport photos, completed application forms, which are available at the post office, and the filing fee -- $55 for adults and $40 for minors.
Girard school levy forum
GIRARD -- A forum to explain the school district's need for a 5.9-mill continuous operating levy will be at 1 p.m. Sunday in the high school gym.
Superintendent Marty Santillo and Mayor James J. Melfi will speak, and the Girard Community Band will perform.
Fatal head-on crash
KINSMAN -- The Ohio State Highway Patrol is investigating a head-on crash that claimed the life of a 37-year-old Kinsman woman.
The patrol said Brenda L. Williams of Garden Street was driving north on state Route 5 around 10:49 p.m. Wednesday, when her vehicle went left of center and struck a car driven by Rodger L. Mellon, 56, of Russel Street, Kinsman.
Williams, who was not wearing a seat belt, was pronounced dead at the scene. Mellon was treated at a Youngstown hospital for injuries.
Southington crash hurts 3
SOUTHINGTON -- A two-vehicle crash Thursday sent three people to the hospital.
The Ohio State Highway Patrol said Lisa Costianes, 28, of Phalanx Mills Herner Road, Southington, was driving south on Phalanx Mills around 3 p.m. Thursday when her vehicle collided with a car driven by Karla Veon, 39, of Crestwood Drive, Garresttsville.
Costianes and her 3-year-old daughter were flown to Cleveland Metro Health Center, where both were in serious condition, officials said.
Veon was treated at a Warren hospital for injuries.
Road rage assault
MERCER, Pa. -- Road rage led to a confrontation Thursday between a Brookfield man and a Hubbard man, according to Pennsylvania State Police.
Police said Dennis B. Clark was apparently angered by the driving of Mark Kwiecinski of Hubbard as their vehicles traveled along U.S. Route 62 around 12:20 p.m. Thursday.
Clark followed Kwiecinski into the Flynn's Tire and Service Center parking lot, about four miles east of Mercer, started arguing, then returned to his truck and got a shotgun, police said.
Kwiecinski struggled with him for control of the shotgun, police said.
Clark fled, but was stopped and arrested. He was arraigned on a charge of aggravated assault before District Justice William Fagley, who freed him on $25,000 bond.
Family's home burns
WARREN -- A child playing with a lighter started a fire, causing damage to a home at 609 Buckeye Street N.W. about 4:30 this morning, firefighters said.
Fire Chief Ken Nussle said the occupants, Tina Robinson and her children, were out of the two-story wooden-frame house before firefighters arrived. No injuries were reported.
The Trumbull County Chapter of the Red Cross provided Robinson and her family with housing.
Gustavus tanker truck
GUSTAVUS -- Gustavus Volunteer Fire Department received a $135,000 federal grant to buy a new tanker truck.
The Assistance to Firefighters Program grants are designed to help fire departments in operations, health and safety programs, new equipment purchases and emergency medical service programs.
Gustavus is responsible for matching 10 percent of the grant, or $13,500.
Holidays at Hoyt
NEW CASTLE, Pa. -- Hoyt Institute of Fine Arts, 124 E. Leasure Ave., will be the setting for the annual free holiday preview from 10 a.m. to 5 p.m. Saturday and noon to 3 p.m. Sunday.
Silk floral arrangements, decorated Christmas trees, centerpieces, gift items, jewelry, handbags and other home decor items will be displayed in five rooms, and gourmet food tasting will be available.
Dog missing after burglary
SALEM -- A female tan Sharpei, valued at $800, is missing after a burglary Thursday night at a house in the 5200 block of Salem-Unity Road. The dog's owner told a Mahoning County deputy sheriff someone entered the house through an unlocked door, used lipstick to write an obscenity on the TV, scattered items in the living room and dumped a bottle of beer on a chair. It's not known if the dog was taken or got out.
