Some of the changes to Columbiana County employees' health insurance plan. The new rates take effect Sunday. Employees do not pay monthly premiums.
DEDUCTIBLES
Individual: From $125 to $300 network; from $200 to $500 non-network.
Family: From $250 to $600 network; from $400 to $1,000 non-network.
OUT-OF-POCKET MAXIMUM (INCLUDES DEDUCTIBLE)
Individual: From $625 to $1,500 network; from $1,700 to $2,500 non-network.
Family: From $1,250 to $3,000 network; from $3,400 to $5,000 non-network.
PRESCRIPTION DRUG CO-PAY
Retail (maximum 30-day supply): From $10 to $20 for brand names; from $5 to $10 for generic.
Mail order (maximum 90-day supply): From $16 to $32 for brand names; from $8 to $16 for generic.
