Information on where you can get flu shots.

Trumbull County

Provider: Niles City Health Department. No charge. Those planning to get vaccinated are encouraged to register for the vaccine online at www.h1n1vaccine.odh.ohio.gov or by visiting www.nilesh1n1.com Web site for preregistration information. Please bring the printed consent form at the time of vaccination. People also will be able to fill out consent forms at the H1N1 flu clinic. Dec. 3: 2 to 6 p.m. no appointment needed, at the Mayor Ralph A. Infante Wellness Center, 213 Sharkey Drive, Niles.

Provider: Trumbull County Health Department, along with Newton Falls Exempted Village Schools, has scheduled the district’s H1N1 swine-flu clinics. Only students with signed permission forms will be given the vaccine. Parents who wish to accompany their child will be directed to specific waiting areas. Parents will not be permitted to enter classrooms.

Dec. 4: 1 to 5 p.m. at each of the district’s three school buildings.

COLUMBIANA COUNTY

Provider: Columbiana Health Department will be administering the vaccination with injections at Beaver School District for schoolchildren. Be sure to complete registration form in advance, and it must be signed by parents or guardians. It can be found on the Beaver Web site: www.beaver.k12.oh.us. If you do not have access to the Internet, you may pick up a form at any of the district buildings.

Monday: 2:45 p.m. at high school only. Students in grades K-4 must be accompanied by their parents and/or guardians. Parents of elementary students may pick up their children at their respective buildings at 2:30 p.m. Park in the south lot of the high school, and enter through the gymnasium doors. The clinic will be open until 6 p.m. During the day, clinics will be set up at the middle school and high school to vaccinate students in grades 5 through 12.
