A. Strangely enough, you must make the same bid whether you are playing two-over-one as a game force or as an invitational bid. In the latter case, you must jump to four hearts to show game-going values. In the former, you must use the Principle of Fast Arrival to show a minimum game force with no slam interest.

A. It might seem automatic to rebid your good six-card major, but suppose you do and partner bids three no trump. What now? If you bid four spades will partner believe you hold three-card support headed by the ace? At the table, a world champion avoided this problem by raising to three spades now, concealing the sixth heart for the moment, and an excellent spade slam was reached.
