Accident date: July 9
Description: Structural failure on the Son of Beast roller coaster. Twenty-seven people transported to local hospitals. Two admitted at a Cincinnati hospital with upper body injuries.
Findings: ODA Enforcement Division ruled structural failure.
KALAHARI
Location: Kalahari Resort, Sandusky
Accident date: March 1
Description: A 10-year-old girl suffered head trauma on the water/tube slide. She collided with a ride attendant.
Findings: ODA Enforcement Division ruled attendant error. Civil penalty issued.
AIRPORT GO KARTS
Location: Cleveland
Accident date: July 29
Description: A 10-year-old boy was involved in a collision with another go-cart. He received first aid for a cut under his eye. He was later taken to a hospital and diagnosed with a concussion and was admitted.
Findings: ODA Enforcement Division ruled patron error, saying the boy entered the pit area too fast, bounced off the rail, and was hit from behind by another go-cart.
BATES AMUSEMENT CO.
Location: Lorain County Fair
Accident date: Aug. 23
Description: A 12-year-old girl was shocked while leaving the round-up ride. She was transported to a Cleveland hospital and released the next day.
Findings: ODA Enforcement Division ruled that insulation had worn off the electrical wires and caused the ride to be charged with current.
